I want to inform you that Silverman Ankle & Foot will be closing, effective March 2, 2025, as I will be relocating to Atlanta, GA. It has been a privilege and an honor to have served as your Orthopedic Surgeon in the Twin Cities for the past 20 years.
Upcoming Appointments:
- If you have an appointment scheduled in the coming weeks, we will discuss your transition to a new physician during your visit.
- If you do not have an upcoming appointment but wish to begin the process of finding a new provider, our office is facilitating transitions to Dr. Brian Sleasman, MD at Allina Health. Dr. Sleasman is an esteemed Orthopedic Foot and Ankle Surgeon. You can contact his care coordinator, Jessica at (651) 241-6640.
Choosing a New Provider:
- While we recommend Dr. Sleasman, you are free to select any provider of your choice. Please make sure to establish care with your new provider.
Medical Records:
- After the office closes, your medical records will be transferred to Morgan Records Management, LLC. They will maintain your records in compliance with all applicable confidentiality, security, and legal standards.
- Record Retention: Your records will be preserved for a minimum of seven years after your last service date, in line with state and federal laws.
- Requesting Records: You can request copies of your records to be sent to you or another provider either now or after the closure by contacting:
- Online: MorganRecordsManagement.com > Medical Records Requests > Request My Medical Records
- Email: Medical@MorganRM.com
- Phone: (833) 888-0061
Please Note:
- There might be a nominal fee for transferring records as per state law.
- During the next three months, Morgan Records Management will have full access to our records.
If you have any questions or need help finding a new provider, please contact our office.
I deeply appreciate the trust you have placed in me and our office and extend my best wishes for your continued health, happiness, and success.
Sincerely,
Lance M. Silverman M.D. F.A.A.O.S.
