2. Complete Paperwork Prior to your First Visit
At Silverman Ankle & Foot, we understand the tedious nature of completing paperwork in the office. To improve the experience of your visit, please pre-register with our electronic health record and complete our New Patient paperwork prior to your visit.
Please print out and complete each of the following forms and send by mail to our Edina office (6600 France Ave. S., Edina, MN 55435) or fax us at 952-224-8515 at least one week prior to your visit. We will confirm receipt of the forms and the appointment by your preference of email or phone. If we have not received your forms in sufficient time prior to your appointment, you will need to complete them in the office. And, as unexpected things can happen, we suggest bringing a copy of the completed forms with you the day of your appointment. Please click all of the links below to download each form.

3. Share your Prior Medical Records & Imaging Films
If you have been evaluated or treated for this problem by another health care provider, please call their office and request a copy of your medical records and films. Although it is more convenient to have them sent to us, it is infinitely more reliable if you pick them up and bring them to your appointment. You may fax paper records to: 952-224-8515.
Please bring to your appointment all available previous films including: X-Ray, MRI and CT scan (Preferably on a Disc). If you have had surgery on your ankle and/or foot, a copy of your surgeon’s operative report is invaluable in this visit.

Personalized Care Doesn’t Always Follow Set Time Schedules – Your Understanding is Appreciated
The care of each and every patient is the highest priority of Dr. Silverman and his staff. Because he takes that commitment very seriously, extra attention beyond scheduled times sometimes becomes necessary. We know you appreciate that special care when you’re the recipient of it. We appreciate your patience and apologize should you experience any long wait times or delays. Please know that we do our best to see you as soon as possible and make sure you receive individualized attention.
